Site Information
Complete the forms below to edit the site information. The site name will be the title of the page. The start and end dates indicate how long the site will be published and accessible to commenters. The return URL refers to the page the site will redirect to after the commenters submit their comments.

The following instructions will always appear at the top of the page: “Instructions: Individuals or groups submitting comments will be asked to select the appropriate heading/subheading for which they would like to make a comment. All indices (drop-down lists) must be selected before the comment can be submitted.

Each page allows for five separate entries; if the submitter is interested in making additional suggestions, there will be the option to return to the submission screen after the first five entries are submitted. The submitter’s personal information will still be captured at the top of the screen if there are multiple submissions as long as submitter has not closed the browser.”

Uploaded files will be provided to commenters on which they may comment. At least one file must be uploaded. Files should be in PDF format and may be up to 5MB in size. Please be sure to name files on your computer before uploaded, as they cannot be renamed.

For example, comments can fall under chapters in the document or by topic (General, Training, Professional Practice, Research and Advocacy). To add sections, complete the forms below. The minimum number of sections is 1 and the maximum number is 4.
